Forney medical staff biographies:
Lawrence Bean,
MD
Dr. Bean received his undergraduate degree in Biology, from
the University of Texas at Arlington. He received his medical
degree from the University of Texas Southwestern Medical Center
in Dallas and completed a Medical Internship and Residency
at the prestigious and rigorous Parkland Memorial Hospital/University
of Texas Southwestern Medical School. Dr. Bean is a board
certified emergency Physician and is a member of the American
College of Emergency Physicians, Texas College of Emergency
Physicians and the Texas Medical Association.
Dr. Bean was born in Oxnard, CA. He has lived in the Dallas / Ft Worth area for
24 years. He served as a firefighter for the city of Garland, TX for over 8 years
before becoming a physician. He has lived in Forney for over 14 years where he
currently resides with his wife and four sons. He enjoys camping, fishing and
spending time with his wife and kids.
Javier Caldera-Nieves, MD – Director of Laboratory
Services
Dr. Caldera received his undergraduate degree in Biology from
the University of Puerto Rico. He received his medical degree
from the University of Puerto Rico School of Medicine and completed
a medical internship and residency at the University of Puerto
Rico Hospital at Carolina. Dr. Caldera went on to complete
a 2 year fellowship in Toxicology at Parkland Memorial Hospital/University
of Texas Southwestern Medical School in Dallas. He is Board
Certified in Emergency Medicine, Board Eligible in Toxicology,
and is a member of the American College of Emergency Physicians,
Texas College of Emergency Physicians and the Texas Medical
Association.
Dr. Caldera, a native of Puerto Rico, has lived in Dallas for 3 years with his
wife and their 2 daughters. An avid gardener he enjoys growing bonsais and also
raises saltwater and koi fish.
Eugene Gicheru, MD
Dr. Gicheru received his undergraduate degree in Chemistry from
the University of Texas at Austin. He received his Medical degree
from the University of Texas Medical Branch at Galveston. He completed
a medical internship and residency at the prestigious and rigorous
Parkland Memorial Hospital/University of Texas Southwestern Medical
School. Dr. Gicheru is Board Certified in Emergency Medicine and
is a member of the American College of Emergency Physicians, Texas
College of Emergency Physicians and the Texas Medical Association.
Dr. Gicheru was born in Nairobi, Kenya. He lives in the DFW Metroplex
with his wife and three year old son. He enjoys camping, water
sports, travel, electronic gadgetry and spending time with his
family.
Shilpi Mittal, MD – Medical
Director
Dr. Mittal received her medical degree from Jawaharlal Nehru Medical
College in India. She completed a Medical Internship at St. John
Medical Center in Detroit, Michigan, and her residency in Family
Practice at the Goppert Family Practice, Baptist-Lutheran Medical
Center in Kansas City, Missouri. Dr. Mittal is Board Certified
in Family Practice and is a member of the American Academy of Family
Medicine, the American Medical Association, and Southwest Clinical
Society of Family Physicians. She has worked for a large outpatient
practice in Missouri where she also held a faculty position teaching
new physicians.
Dr. Mittal was born in India and immigrated to the United States.
Although she lived in Missouri she got to Texas as soon as she
could! She is married with 2 wonderful sons and enjoys reading,
traveling and gardening.
Daniel
Surdam, MD
Dr. Surdam received his undergraduate degree in biology from Colorado
College, Colorado. He received his medical degree from Creighton
University School of Medicine in Omaha, Nebraska and completed
a medical internship and residency at the prestigious and rigorous
Parkland Memorial Hospital/University of Texas Southwestern Medical
School. He is Board Certified in Emergency Medicine and is a member
of the American College of Emergency Physicians, Texas College
of
Emergency Physicians and the Texas Medical Association.
Dr. Surdam was born in Laramie, Wyoming. He lives in Cheyenne, Wyoming but continues
to collaborate with physicians in the DFW Metroplex. He enjoys outdoor activities,
golf, traveling, SCUBA, and home improvement projects.
Alex Jordan, RN, BSN
Alex completed his undergraduate work at the University of Texas
at Arlington, earning a Baccalaureate degree in Nursing. Thereafter,
he completed a Nursing Internship with Methodist Health System
in Dallas. He is a Certified Emergency Nurse and is a member of
the Emergency Nurses Association. Alex has an extensive background
in Emergency and Critical Care Nursing and has worked for several
large hospitals in the DFW area.
Alex was born in Dallas and has lived in the DFW Metroplex all of his life. He
enjoys cycling and traveling, and spending time with his son.
Janet Barnes,
RN – Director of Occupational Medicine
Janet received her associate degree in Nursing from Tarrant County
College and completed her Critical Care Internship with Methodist
Hospital of Dallas. She is a member of Emergency Nurses
Association and has worked at some of the busiest Emergency Departments
in the DFW Metroplex. She also has extensive training in Occupational
Medicine and works closely with employers to ensure their injured
workers receive quality and efficient care.
Janet was born in Lawton, Oklahoma but has called North Texas her home since
1988. She is married and has three beautiful children. She enjoys camping, fishing
and spending time with her family.
